Coaching Log Consent

Coaching sessions are conducted in accordance with the International Coaching Federation (ICF) Core Competencies and Code of Ethics.

Prior to our first coaching session, I will require your written consent to the collection, use, retention, and disclosure of your personal information (full legal name, email address, and phone number) to the ICF solely for credentialing and coaching log verification purposes in the event of an audit. Upon receiving your consent, I will provide my reciprocal written consent for the same purposes.
